/>Should Smokers be allowed Free Medical Treatment?</p><p>With the health service in the state that it is &#8211; should smokers be responsible for the costs of their own health care?</p><p>Everyone has freedom of choice and smokers can make the choice to stop, but by continuing to smoke they are creating their own health problems. But is it right that non-smokers should have to subsidise health care treatment that prolongs a lifestyle of slow suicide? Why should people who do look after their health still have to indirectly subsidise the smokers&#8217; tobacco habit through their National Insurance contributions?</p><p>Smokers say they want the free choice of being able to smoke but they do not appear to want to fully accept all the consequences of smoking. They still want someone else to subsidise their cancer or heart disease treatment. And what about secondary smoking? If you take drugs</p><p>or alcohol, and then hurt someone whilst under the influence of these, you are answerable to the law. Through secondary smoking, smokers can give cancer to non-smokers including children; shouldn&#8217;t they also be answerable to the law?</p><p>If smokers were forced to take responsibility for their own health by having to purchase private health care, this would release funds and facilities in the National Health Service for the treatment of the general public with non self-inflicted health problems. Perhaps then the health service would not be in the state it is in. And let&#8217;s not forget the other costs involved: not only do smokers pollute the atmosphere with smoke and ash which increases the cost of keeping buildings clean and fresh, they also litter the ground with cigarette ends, increasing the cost of street cleaning. Then there are the still-burning cigarette ends, carelessly thrown away, causing fires and damaging crops, thus increasing the burden on the fire service and farmers.</p><p>Some smokers do appear to be aware of the health risks they take and believe they should take responsibility for their own health &#8211; which is why they would like to stop smoking; but whilst smoking remained socially acceptable in pubs and restaurants, and their friends were lighting up, they found it extremely difficult to do so. It does appear that a majority of smokers genuinely welcome the ban on smoking in public buildings, as it will help them to stop smoking due to the temptation no longer being there when their friends light up. Non-smokers will also be able to enjoy a smoke-free night out and not have the really rough feeling next day with their clothes and hair stinking of smoke.</p><p>Those who do continue to smoke are now social outcasts and should look at the reasons why they are smoking. They need to take responsibility for their own health, placing the health care treatment burden of their self inflicted cancer or heart disease on themselves, instead of non-smokers having to subsidise them. People are usually misled by the fact that cigarettes are sold legally and they are not fully aware how dangerous, addictive and fatal they can be. Smoking causes various truly harmful diseases; from various types of cancer, to heart and cardiovascular diseases and infections.</p><p>Here is a list of most common diseases smoking causes</p><p>1. Cancer</p><p>Smokers unfortunately have an extremely high risk of getting some type of cancer. The reason why this happens is simple: carcinogen is present in tobacco and that is a substance which causes various kinds of cancer: lung cancer, mouth and tongue cancer, and throat and gullet cancer. These diseases are almost always terminal and practically impossible to cure. More than 85% of patients with diagnosed lung cancer for instance, are smoking regularly. Unfortunately, second hand smokers are not protected as well, so they often end up getting sick and that is a real tragedy. There are other types of cancer also related to smoking: pancreatic and bladder cancer, and cervical cancer (female smokers).</p><p>2. Cardiovascular diseases</p><p>Cardiovascular diseases include heart malfunctions, problems with blood vessels, clogged arteries etc. They are a huge health problem and the major cause of death among smokers. COPD a.k.a Chronic Obstructive Pulmonary Diseases</p><p>COPD is a large group of diseases which cause various breathing difficulties. Bronchitis (regular or chronic), emphysema and shortness of breath are in this category. Once you start smoking and inhaling smoke, you cause damage to your air sacks (or alveoli) and that is the main reason why smokers often suffer from these diseases.</p><p>There are other, numerous health conditions and diseases, caused by smoking. All those chemicals present in tobacco are the main reason why smokers often get sick and suffer from all these diseases. Smoking is hazardous for your oral health: teeth suffer and gums often experience problems and soreness. Mouth ulcers are also very common as well as other infections. Skin aging should also be taken seriously, because that is a direct consequence of smoking. Someone in the family has to spearhead the change. It is usually the task of the family cook and shopper. He or she has to accept the fact that it will be an unpopular move. When the cupboard no longer contains sweet biscuits and the refrigerator is empty of sugary drinks there will be accusing eyes and wailing voices. Be strong. Make sure there are substitutes so that the cry &#8220;there&#8217;s nothing to eat&#8221; doesn&#8217;t ring in your ears. Give them chilled strips of carrot and celery always on tap. Have delicious fruit cut up and ready for the after-school onslaught. Be inventive about snacks. Cook books always have answers. If we cut sugar down to a minimum and use whole food ingredients snacks are possible. If you are trying to be a vegetarian household do it gradually and don&#8217;t necessarily tell them the lentil hamburgers have breadcrumbs instead of mince meat. Chances are they won&#8217;t notice on a plate packed with vegetables and sauces. One woman disguised her nutritive sauces by saying in answer to a suspicious&#8217;:What&#8217;s that!&#8221; &#8220;Pink stuff,&#8221; and then changing the subject.</p><p>It is hard to change eating habits particularly if the normal diet has had a lot of salt in it. It is essential to make the substitute fare delicious and worth eating. One nutritionist wrote recently that after years of pleading with his family to change their diet he tried asking them to do it just for 24 hours. When they did they found themselves feeling much better and grudgingly agreed that he might have a point. But if we love our families we want to head off diabetes and cancer and all the other horrors that come with eating the wrong food filled with additives and salt and so we need to persevere. If it&#8217;s too hard to do it all at once then try it on alternate days until you gradually change entirely to the diet that is healthy Afternoons and late evenings are the times when most snacking occurs and it is almost always involved with television. So try to eliminate that habit too. Try taking children to a park in the afternoon for a game that you can enjoy too. Or take them to swim or skate but not anywhere selling ice creams or drinks. At night try a board game that will intrigue them away from the advertisements going on relentlessly on television. The payoff comes in slimness and health and a great sense of achievement and satisfaction. Tobacco helps in generating revenue for different governments ranging up to 12% of their total GDP and is great employer1. It has a definite place in adolescent mind as a tool for style status. It is a matter of adult choice. Tobacco is a legal product, but tobacco is injurious to health. Smoking is an increasingly important problem in the World. Millions of people have used or currently use and abuse, illicit drugs. Substances most likely to be abused include agents that cause euphoria and/or provide relief from stress or boredom2.</p><p>SUBSTANCES OF ABUSE3</p><p>1. Commonly available</p><p>Â· Nicotine</p><p>Â· Alcohol</p><p>2. By prescription only</p><p>Â· Amphetamines</p><p>Â· Barbiturates</p><p>3. Illicitly used</p><p>Â· Morphine</p><p>Â· Cocaine</p><p>Â· Heroin</p><p>Â· Hashish, ganja, marijuana</p><p>CAUSES OF SUBSTANCE ABUSE</p><p>1. Physiological</p><p>2. Genetic, and/or</p><p>3. Environmental factors</p><p>Cigarette smoking is the most preventable cause of illness and premature death in India and the whole world. Nicotine, a volatile liquid alkaloid, is the constituent of tobacco that causes dependence. The amount of nicotine absorbed is varies according to the type of tobacco used, the length of the cigarette smoked, and the presence and type of filter used. Tobacco exposure also depends upon the number of cigarettes smoked per day and the amo<div

The drug increases catecholamine release from peripheral adrenergic neurons. It also increases respiratory rate, coronary blood flow, heart rate, and blood pressure. Salivary and bronchial secretions are initially increased but later decreased. CNS depression follows the initial central stimulation. Nicotine causes a temporary depression of appetite and decreased taste sensations. Cigarette smokers have elevated carboxyhaemoglobine (COHb) levels due to inhalation of excess CO from combustion of tobacco. Because COHb cannot carry oxygen, less oxygen can be transported by the circulatory system. This deficiency may contribute to decreased exercise tolerance among smokers. Respiratory tract disorders and Bronchitis, in general, are more prevalent, not only in smokers, but also among their nonsmoking family members.</p><p>PROBLEMS CAUSED BY SMOKING</p><p>(Incidence higher than non-smokers)</p><p>Tobacco is the cause of serious illness and many premature deaths in both developed and developing countries. Dependence on tobacco is a chronic condition, which is difficult to overcome. The treatment of medical conditions induced by tobacco use is a major cost factor in health care4-7.</p><p>1. Respiratory diseases, e.g. halitosis (bad breath), chronic obstructive pulmonary disease (COPD), emphysema, Lung cancer</p><p>2. Cardiovascular diseases: Hypertension, Heart attacks</p><p>3. Diabetes</p><p>4. Strokes</p><p>5. Peripheral vascular disease and cerebro &#8211; vascular insufficiency</p><p>6. Gastrointestinal (GI) disturbances: Peptic ulcer, epigastric discomfort, gastritis and ulceration of gastric and duodenal.</p><p>7. Wrinkles on the face, discolored fingers and lips, psoriasis</p><p>8. Lack of stamina</p><p>9. Periodontal disease, tooth decay</p><p>10. Impotency and infertility</p><p>11. Cataract</p><p>12. Uterine cancer and miscarriage</p><p>Cigarette smokers tend to die at an earlier age than non-smokers. Every year four million people die because of the use of tobacco among them eight Lakh Indians. Lung cancer is a major contributor to this mortality. Cigarette smoking causes approximately 70 to 80% of lung cancers. Smoking also increases an individualâ??s risk of the larynx, mouth, urinary bladder, pancreas, cervix, and kidneys. One out of three smokers will die of this habit; half of them in middle age. Average life of a smoker is 7-12 years less than that of non-smokers8-9.</p><p>Smoking during pregnancy is associated with higher than normal rates of miscarriage, spontaneous abortion, prenatal mortality, and premature birth. The newborn of woman who smoke during pregnancy are more likely to be underweight, be short in stature, and have a smaller head2.</p><p>It is also causes immeasurable suffering to humanity through other diseases. It is compounding factor, which adversely affects the outcome of few treatments, e.g. a smoker afflicted with tuberculosis has five times more chances of dying from it than a non-smoker5.</p><p>Other form of tobacco like smokeless tobacco, dipping snuff, cigar and cigarillo are also addictive and produces same effects like tobacco. Even passive smoking kills.</p><p>PHARMACISTâ??S ROLE TO SAY GOOD-BYE TO SMOKING</p><p>Pharmacists, in common with all other health professionals, have a responsibility to help people who wish to do so, to give up smoking or other uses of tobacco and to encourage others to do so. Pharmacists are committed to taking relevant action to eliminate tobacco use in the communities the serve9-11.</p><p>1. Take a more active role by providing services to those who wish to quit the use of tobacco and to those who have tobacco induced disease, the services to include group and individual cessation programme.</p><p>2. Tell addict and give confidence to him that he can quit smoking; provide an example and leadership by being free of tobacco themselves.</p><p>3. Tell never try to smoke in public area.</p><p>4. Lead and participate in media campaign designed to highlight the dangers of use of tobacco and to help people to quit.</p><p>5. Advise when there is great desire to smoke than drink cold water or eat chewing gum (now a dayâ??s chewing gums and transdermal patches available with nicotine and bupropion for smoking cessation).</p><p>6. If s/he a chain smoker then suggest try to reduce the no. of smoking.</p><p>7. Show and make a good picture of him for a future when s/he quit smoking.</p><p>8. Motivate victims to see towards children and their future.</p><p>9. Advise to save time, money and life through activities of â?¾pharmacist action against smoking.</p><p>10. Make them aware that for making 300 cigarettes we need to cut down one tree, it is very much detrimental to environment.</p><p>11.
