Commissurotomy in India: This surgery is recommended for the treatment of mitral stenosis in the heart and to prevent pulmonary hypertension and heart failure. The Commissurotomy procedure: There are two types of commissurotomy that are performed. In a ‘closed’ commissurotomy, a specially designed, deflated balloon (Inoue balloon) on the tip of a catheter is passed through another catheter that has been threaded from a vessel in the groin area up into the opening of the mitral valve. The balloon is then inflated to stretch open the valve and relieve the narrowing or obstruction. The entire procedure is monitored by a special x-ray machine and can be viewed on a monitor above the operating table in the cardiac catheterization lab. Although this is a minimally invasive procedure, it is technically challenging and takes about 4 hours to complete. The ‘open’ or ‘surgical’ commissurotomy, sometimes called a valvotomy, accesses the mitral valve through a chest incision. The surgeon can physically release the partially fused leaflets of the narrowed mitral valve with a finger or mechanical probe. The open procedure allows for direct visualization of the mitral valve and provides the opportunity to correct other valvular defects, such as removing the blood clots or calcium deposits on or around the valve.
